Coverage
Coverage depends on the plan and the use.
Coverage describes what a health plan may pay for, subject to its medicine list and approval rules.
Connect the proposed use to a payment route
Use the exact name and reason for care
Give the plan both the medicine’s name and its proposed use. Request an answer tied to your particular benefits.
- Is this product covered for this use?
- Which forms are included?
- What must be approved before the first fill?
For Medicare, the use changes the route
For type 2 diabetes, tirzepatide (Mounjaro) was eligible for Medicare Part D payment on September 26, 2026. For obesity alongside moderate or severe obstructive sleep apnea, tirzepatide (Zepbound) could also qualify for adult Part D coverage. With this sleep disorder, breathing is blocked repeatedly during sleep. Part D is Medicare’s drug coverage. The plan checks which medicines it includes and whether it needs to approve payment beforehand.
- Ask your plan to confirm whether it will pay before you count on coverage.
A temporary program has narrower terms
The Medicare GLP-1 Bridge set an eligible person’s share at $50 a month for certain weight medicines on September 26, 2026. Its start date was July 1, 2026; the announced end is December 31, 2027. For 2026, Bridge requests go to a central service that decides approval and pays pharmacies. Your usual Part D claims take a separate path. Paying the $50 brings you no closer to meeting your Part D deductible or the yearly limit on your own drug spending. For tirzepatide, the program includes only the Zepbound KwikPen, a pen holding several doses for one patient. Vials, which are small medicine bottles, and single-dose pens are excluded.
- Ask the prescriber and pharmacy how they will arrange the Bridge request and payment before paying.
Who can qualify for this program
The Bridge requires age 18 or older. The weight and health checks look back to the start of GLP-1 treatment, under rules recorded September 26, 2026. GLP-1 is the program’s name for this group of medicines, which includes tirzepatide. The rules use BMI, a score relating weight to height. Ask the clinician which of these groups describes you.
- At a BMI of at least 35, the weight requirement is met.
- At a BMI of at least 30, you also need heart failure, blood-pressure problems, or kidney disease that meets the rules below.
- At a BMI of at least 27, the added condition can be pre-diabetes or artery disease that causes symptoms in your limbs.
- A history of stroke or heart attack also meets that added condition at BMI 27 or above.
Ask the clinician to check the exact heart, blood-pressure, or kidney rule with you. The heart-failure rule is for the type where the heart squeezes out a normal proportion of its blood with each beat. The blood-pressure rule requires readings above 140 for the upper number or above 90 for the lower number, despite two medicines for blood pressure. For kidney disease, the rule requires stage 3a or later; ask the clinician what stage is in your record.
Your diagnoses and drug plan also matter
Type 2 diabetes and moderate-to-severe obstructive sleep apnea are reasons for using ordinary Part D coverage. Having either condition rules someone out of the Bridge. Another exclusion is the liver illness called MASH when scarring is moderate or advanced but has not reached cirrhosis, a severely scarred liver. Prescriptions to reduce major heart and blood-vessel risks also go through ordinary Part D.
- For 2026, the Bridge excludes anyone whose Part D plan has already covered GLP-1 treatment during that calendar year.
- The recorded rules had not yet set how past coverage would affect eligibility in 2027.
- The eligible insurance types include a standalone Part D drug plan, bought separately. Medicare Advantage members need drug benefits through the type called a coordinated care plan.
- Ask your insurer to check the exact plan type with you.
- A prescriber must order the medicine and obtain advance approval. Treatment must include continued support for eating habits and physical activity.
- Ask your plan and prescriber to check every condition with you. Your age alone does not settle whether you qualify.
